Therapeutic approach in a case of complex regional pain syndrome of the upper limb

Abstract

39-year-old patient with a Colles fracture in 2021, managed orthopedically. Subsequently developed dysesthesias and edema, initially diagnosed as a conversion disorder. An EMG revealed distal axonal neuropathy of the right radial nerve, and scintigraphy confirmed posttraumatic CRPS in a hyperemic phase. Diagnosed with CRPS of the right upper limb, treated with lidocaine-ketamine and stellate ganglion block without improvement. In 2023, the patient presented with severe pain, flexion contractures, and extreme allodynia, considering amputation. Hospital admission was proposed for forced mobilization under interscalene block. During the procedure, MCF joint dislocation of the third finger was observed. Lidocaine-ketamine infusion and rehabilitation therapy with botulinum toxin, mirror therapy, and psychosocial intervention were initiated. Favorable evolution was achieved, with progressive functional recovery, allowing the patient to resume work and sports activities.
